Blast monitored was use with a meter with expired calibration. Calibration was not correctly scheduled due to human error. Blast monitor was calibrated on 7/9/2012. Automatic notifications & reminders for upcoming calibration requirements.

Loss of data from Tapered Element Oscillating Microbalance (TEOM) air quality monitoring unit. Cause was electrical fault on sample pump. An electrician will be arranged annually to inspect power distribution at the monitoring station.

2 Tapered Element oscillating Microbablance air quality monitoring units did not start up correctly after power was lost. Due to the start up sequence not being followed. In future the service contractor will be notified to advise correct procedeure.

Data lost from meteorological station. Temperature sensor at 2m failed. Identified by contractor and subcontractor, however no action was taken. Daily checks to monitoring station were implemented to identify any loss of data in timely manner.
